St. Jude Parish Registration Form
This registration form is based on the family model with two parents and up to four children. If this form will be inadequate for your family, please stop by the parish office to pick up a paper form that may be more suited to your needs.
Adults other than the parents of the family living in your household (i.e. grandparents living with their children's families, college-age youth living at home, etc.) are asked to register separately.
